New Patient Registration

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

New arrivals to the area wishing to register with the Practice should do so as soon as possible. This will speed up the transfer of your medical notes from your previous GP.

Please call in to the practice and pick up a registration pack for each family member registering with the practice.  Alternatively, you can print a copy of the full registration pack out below.

Once completed, return the pack/packs to the reception. You may make an appointment for a health check upon registration.

Parking

A large car park is provided at Ash Grove for patient use. Please use the car park at the front of the building. Car park at rear of the building is for Staff only. A bike park is provided at the rear of the building.

(The practice cannot take responsibility for patient's cars you park at your own risk).
