Care Navigation

Our Patient Coordinators are trained in Care Navigation and will direct you to the most appropriate service to be dealt with in the most effective and appropriate way. Care navigation can occupy many roles and plays a crucial part in helping patients get the right support, at the right time, to help manage a wide range of needs.

Our Patient Coordinators will therefore ask you the nature of your problem to enable them to direct you to the most appropriate care for your needs. All members of the GP practice follow data protection regulations and therefore keep all information given confidential.

Care Navigation is about getting the right care, by the right person, at the right time for patients in Knottingley and the surrounding areas.

Care navigation improves access to primary care services for patients. It allows primary care staff to provide patients with information on health and wellbeing services local to their community.

It is designed to give our patients choice to access the most appropriate service, offering our patients some alternatives to having a GP appointment, if it is suitable for them.

Our practice team has a variety of clinicians who are available to help you, this includes GPs, Advanced Clinical Practitioners, Pharmacists, Practice Nurses, Health Care Assistants, First Contact Physio, Mental Health Practitioner, Care Coordinators, General Practice Assistants (GPA), Phlebotomists, Health & Wellbeing Coaches.

Home visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 10am.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Telephone appointments

Are available daily. You may be offered a telephone appointment if you are requiring:

  • A continuation sick note
  • Test results
  • General health advice
  • Medication review
  • Long Term Condition review

Your appointment

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

It is very important if you can not attend your appointment, that you make the surgery aware.  This appointment can then be offered to another patient, who might need it.

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • using the GP online system via the link at the top of this page
  • cancel through PATCHS
  • phone us on 01977 673141, Monday to Friday from 8am to 5pm
  • reply CANCEL to your appointment reminder text message

We operate a very strict policy on missed appointments and you may be asked to leave the list if you miss 3 appointments in a 6-month period without letting us know. The number of missed appointments is displayed in the waiting area to remind all patients how much time is being wasted.
